YOUR APPOINTMENT
- Please take a moment to print and complete the “Patient Forms for First Visit” found on this page before your first appointment. Our office staff can instruct you on the forms you need when you call to schedule your appointment.
- Please bring your insurance card and if coming for Physical Therapy the written order/prescription from your doctor.
- Please bring a copy of your current medications, including dosages.
- Please bring a copy of your surgical/medical history if applicable.
- Please arrive 10 minutes early before your initial evaluation appointment to complete our Automated Check-In and for our staff to copy insurance cards and collect appropriate paperwork.

YOUR APPOINTMENT
We understand that paperwork is no fun and sometimes frustrating. However, as a licensed medical practice, we are lawfully required to provide patients with specific documents. At Caddoo Rehabilitation Associates, we make every attempt to ensure that the process is as easy as possible.
To minimize any delay in initiating your first visit with us, we’ve provided the necessary information and documents that require your signature to be completed before your first appointment. Please review, complete and bring these forms with you.
Also, please bring a list of current medications and dosages and medical and surgical history. Please remember to arrive 10 minutes before your initial evaluation to complete additional medical questionnaires specific to your condition.
